Repeat Prescriptions

If you are a registered patient and take medication on a long-term basis, you can order a repeat prescription in a number of different ways:

  • In person - please pick up a form from reception and hand in to the reception desk.
  • By post – send it to: Story Street Medical Practice and Walk-in Centre, Wilberforce Health Centre, 6-10 Story Street, Hull HU1 3SA                -
  • By fax - to: 01482 225 247
  • Online - please click on the link at the top of this page (registration required).

Please allow two days for us to process your request. You must allow an extra day if you want your prescription to be forwarded direct to a pharmacy ready for collection.

If you require any other medication that is NOT on your repeat medication list then you will need to discuss this with a GP first. The easiest way to do this is by arranging a telephone consultation.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.

Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
